A lengthy delay or absence of orgasm following normal excitation, erection and adequate stimulation. When a male experiences significant delay or total absence of orgasm following sexual activity, he may have male orgasmic disorder. In order to be qualified as a disorder, the symptoms must present a significant problem for the individual.

Mitigating factors include age and adequacy of the stimulation. Orgasmic disorders may be due to a physical disorder or use of a substance (eg, alcohol, opioids, antihypertensives, antidepressants, antipsychotics).

Diagnostic criteria for Male Orgasmic Disorder

  1. Persistent or recurrent delay in, or absence of, orgasm following a normal sexual excitement phase during sexual activity that the clinician, taking into account the person's age, judges to be adequate in focus, intensity, and duration.
  2. The disturbance causes marked distress or interpersonal difficulty.
  3. The orgasmic dysfunction is not better accounted for by another mental disorder (except another sexual dysfunction) and is not due exclusively to the direct physiological effects of a substance (e.g., a drug of abuse, a medication) or a general medical condition.

What are the Cause of Male Orgasmic Disorder

The cause of this dysfunction is rarely physical although it is sometimes confused with retrograde ejaculation. Retrograde ejaculation is when the man ejaculates into his bladder instead of out the urethra. More often than not, the cause is a traumatic sexual experience, strict religious upbringing, hostility, over control, or lack of trust.

Differential Diagnosis

Depression is a common cause of orgasmic difficulty as well as decreased desire and arousal. Problems with achieving orgasm are usually linked to difficulty in developing sufficient arousal. The man's emotional state can inhibit arousal and orgasm.

However, arousal may be sufficient to achieve partial or full erection but not to produce orgasm. Erectile failure may cause more distress than orgasmic difficulty.

Treatment of Male Orgasmic Disorder

Typically once a medical cause is ruled out, working through the underlying issues is very helpful.  Some therapists also use behavioral techniques such as sensate focus which is a more direct approach if underlying issues are not significant.

  • Male orgasmic disorder may caused because of some diseases, psychological cause, intake of drugs and alcohol or smoking, upsetting sexual experience, antagonism, medicines like antidepressants.
  • Male orgasmic disorder is less common as compared to female orgasmic disorder and it may give rise to distress or interpersonal problems.
  • It may cure by with behavioral techniques, psychological study and counseling, sex therapy, strong lifestyle, accurate exercise and a balanced diet.
